Membership Application

I, the undersigned, being fully qualified by special instruction, military service, vocational occupation or strong personal interest, herewith make application to become a member of the above Uniformed Unit of India Shrine. It is fully agreed and understood that I make application for membership in the above designated Uniformed Unit under the following conditions:

1. That I am a member in good standing of India Shrine with dues paid for the current year.
2. That I will, at all times, conduct myself in such a manner as to reflect a desirable image of the Shrine, of the Unit and of Masonry.
3. That I will maintain a primary interest toward the success of the Unit by a harmonious, friendly and cooperative spirit of participation.
4. That I will attend all Ceremonials, Unit meetings, parades, Shrine sponsored meetings and special programs unless specifically excused by my Commander, Director, Captain or authorized Unit personnel.
5. That I will enter the India Shrine Circus advertising and Ticket sales programs with a conscientious effort to obtain results, limited only by my ability or occupational jeopardy.
6. That I will, at all times, comply with the requests of the Appointed Head of the Unit to the best of my ability.
7. That I know of no physical defects that would prevent me from functioning in the Unit of my choice.
8. That I will pay the annual dues and furnish uniform or special equipment that is required by the Unit, to the satisfaction of the Captain/Director/Commander.
9. That in consideration of my contribution to India Shrine, as a member of a Uniformed Unit, by compliance with the stipulations as herein set forth, the Shrine shall grant certain recognition to be specifically expressed or allocated at the decretion of the Illustrious Potentate.
10. That my tenure as a member of a Uniform Unit of India Shrine shall be contingent upon the will of the Potentate and that he shall retain the full right and prerogative to terminate or limit my membership in said Unit, with or without explanation and without recourse on my part.
